Housing advice for patients experiencing a Delayed Transfer of Care (DTOC)

A patient is considered to be experiencing a Delayed Transfer of Care (DTOC) when they are clinically ready to be discharged but are still occupying a hospital bed due to a lack of suitable post-discharge housing and/or care options.

We aim to support:

  • Anyone struggling to return home from hospital due to their accommodation no longer meeting their needs.
  • Advice on staying put, or moving on from your current home.
  • Advice on financial support to maximise income.

We can also provide:

  • A list of accommodation tailored to your needs.
  • Assistance with housing application forms and registration.

Referrals can be made by medical staff, social care staff or the patient or family members.
